We have a strict No Show appointment cancellation fee policy.
When you do not show up for a scheduled appointment, it creates an unused appointment slot that could have been used for another patient.
It is very important that you call our office at 801-272-3909 (Do not email us or leave a message on our voicemail) a minimum of 24 hours prior to your scheduled appointment to cancel or reschedule.
After three consecutive no–show occurrences, the practice may elect to terminate our relationship with you.
The No-Show Fees are as follows:
For Dr. Luikenaar
- Established patient – $25
- New patient – $50
- For Electrolysis – Equivalent to your Scheduled fee
